LOUIS E. COSTELLO, M.D.
Louis E. Costello, M.D. is board certified in Child, Adolescent, and Adult Psychiatry and has been in practice in the DFW, North Texas area since 1996. He has been practicing in his current location in Highland Village, Texas since 2012. Dr. Costello trained at University of Texas Southwestern in Psychiatry, and he is a Fellow in the American Psychiatric Association. He is recognized for his continued dedication to excellence in clinical care and makes every effort to use the most up-to-date psychiatric assessment, medication management and treatment strategies which are conservative, compassionate, and research-based.
Dr. Costello’s mission to provide the highest quality medical care to his patients, he endeavors to create a professional and calming environment at The Costello Clinic resulting in the most comprehensive and individualized psychiatric care which includes the following: evaluations, referrals to other medical and psychological caregivers, individual therapy and parent and family guidance. Dr. Costello’s goal is for each patient to become as reasonably joyful and functional as their individual strengths will allow them to be.
Meredith Hake, RN, FPMHNP completed her Master of Science in Nursing in 2007, and is a Board Certified Family Psychiatric Mental Health Nurse Practitioner. She received an Honors Bachelor of Science in Nursing degree from the University of Texas at Arlington, and a Bachelor of Arts in Psychology and Spanish from the University of Texas at Austin. She has spent the last 10 years working extensively with patients of all ages with depression, anxiety, autism, PTSD, bipolar disorder, ADHD, and other mental health disorders. Prior to completing her Master’s degree she worked for 8 years with children and their families at Cook Children’s Hospital in Fort Worth, TX. Meredith has worked with Dr. Costello at The Costello Clinic since 2010.
Outside of work, Meredith enjoys spending time with her husband and two children. She is actively involved with her children’s school as well as her church and her community. She is also involved with the Nursing Program at the University of Texas at Arlington, working to help train future Psychiatric Nurse Practitioners.
Donald Van Hooff, RN, FPMHNP is a Board Certified Family Practice Mental Health Nurse Practitioner, graduating from nurse practitioner training from the University of Texas at Arlington in 2005. He began working with The Costello Clinic in 2009. He has over 30 years nursing experience, working with children, adolescents, and adults, and has been providing comprehensive mental health services through collaborative care with Dr. Costello since 2005. In addition to the years of experience as a Psychiatric Nurse Practitioner providing care for patients with a wide range of issues, Don is dedicated to helping individuals with difficult to treat or medication resistant MDD to become as completely well as possible.
Don’s other interests include volunteering in third-world countries, playing chess, and watching foreign films. He views his decision to become a Mental Health Nurse Practitioner as one of the best he has ever made. His thoughtful dedication to his profession and his patients helps drive Don to help each individual under his care.
Patricia Brooks, RN, PMHNP
Patricia is a Board Certified Psychiatric Mental Health Nurse Practitioner caring for patients of all ages. She is the newest member of The Costello Clinic, joining the team in 2018. She graduated with a Master of Science in Nursing from University of Texas at Arlington and received her Bachelor of Science in Nursing from University of California Irvine, graduating Magna Cum Laude. Prior to entering the field of psychiatry, she spent most of her nursing career working in the Neurological Intensive Care Unit, caring for patients with strokes or traumatic brain injuries, and supporting their families.
Patricia enjoys spending time with her family when she is not working. She has also served as Secretary for the Psychiatric Mental Health Nurse Practitioner Association of North Texas and as an area representative for the Psychiatric Mental Health Nurse Practitioner Association of Texas.
Cynthia, Practice Manager
Cynthia is The Practice Manager for The Costello Clinic. She recently moved to the DFW area from her home state of Mississippi where she was the Office Manager for Kindred Home Health and Hospice, formerly Gentiva Hospice, for the past 5 years. Cynthia graduated with a degree in Accounting from Mississippi State University and brings to The Costello Clinic over 30 years of medical office experience including working in the business departments of several medical clinics including The Surgery Clinic of Tupelo. Cynthia has worked for two hospice companies, North Mississippi Residency Center, and also served as a K-5 Health Educator for 10 years with The Community Health Department of North Mississippi Medical Center. She is an amazing addition to our team here at The Costello Clinic, as she just makes everything better!
Cynthia enjoys being of service to patients of all ages and is also passionate about being an advocate for several organizations including The Alzheimer’s Association, The Diabetes Foundation, The Susan G. Komen Race for the Cure, and The Humane Society.
Suzanne, TMS Coordinator and Certified TMS Technician
Suzanne has served as a Certified TMS Technician and as The Costello Clinic TMS Coordinator since 2014. With a Marketing degree from Texas State University and a 25 year career in sales and marketing with four Fortune 100 Corporations, Suzanne brings a different perspective to The Costello Clinic. As the TMS Coordinator she works every day to help educate patients regarding how TMS treatment works. If patients are using insurance, she then ensures the patient’s insurer is aligned with prescribed treatment plan. Once patients are approved, Suzanne is an integral part of the healing process for our patients as she is in the treatment room with them facilitating the CBT programming we use with each TMS patient. Her contagious enthusiasm, love for life and care for our patients brings an added value to our TMS services! Suzanne loves her job, because she is able to physically watch patients become free of depression in just 36 quick treatment sessions!
In her spare time Suzanne enjoys reading and organic vegetable gardening. She is a bee-keeper and also maintains several composting earth worm farms. She is very involved in her church and active in our community. Suzanne loves to travel and has attend the International Clinical TMS Society meetings in Atlanta, San Diego, New York, and Vancouver.

Kaitlyn, Front Office Assistant
Kaitlyn is an intern here at The Costello Clinic. She is currently completing her Bachelor’s Degree in Exercise Sports Science from Texas State University in San Marcos, TX. Kaitlyn has worked in an array of fields including customer service and marketing and she hopes one day to pursue a career in medical sales. In her spare time Kaitlyn enjoys exercising in a variety of different settings and spending time with her family and friends.
As an invaluable asset to our clinic, Kaitlyn checks our patients in and out and assists in other departments as needed. Kaitlyn is excited to be a part of the team here at The Costello Clinic as it has been an eye-opening experience for her to witness the benefits of psychiatric care and the impact of treatments on patients.
Katie, Front Office Assistant
Katie joined our team in the fall of 2018. Katie has been in the medical field since she graduated high school, although being in a psychiatric office was new for her, healthcare is not. She has worked in the front office setting in the areas of Cardiology, Nephrology and OB/GYN. Katie is interested in becoming a certified medical assistant, as well as continuing her learning in natural medicine, muscle testing and psychology. Currently, Katie is working as part of the front office team and enjoys connecting with patients here. Her favorite part of working in our office is seeing patient’s confidence restored in their smile after treatments.
In Katie’s spare time she likes to be out at her family’s farm and exploring new adventures. Katie is an asset to The Costello Clinic by checking patients in and out, verifying insurances and answering any questions that may arise.
